Health Waivers
Health Waivers allow team members to declare their health prior to every game and practice.
All CRMHA participants and spectators are required to fill in a event health waiver form prior to each event. Once complete, coaches, managers and league administrators can track waiver form completion.
CRMHA Has Created a Form
The health waiver form is already set up. Prior to each event, every member of your team will be notified to complete it.
Complete the Form
Participants & Spectators will be required to fill in the form prior to each event. The form is automatically added to each event in every team’s calendar!
Track Completion
Team admins and League admins can view reporting on every event for who attended and if they completed the waiver form.
